Understanding Post and Core Buildups


In prosthodontics, the term "post and core" refers to a two-part restorative process used to rebuild a tooth that has lost significant structure. The technique combines two key components:

Post


The post is a structural element placed within the tooth's root canal, typically after endodontic (root canal) treatment. The post serves as an anchor for the buildup material.

Core


The core refers to the material used to replace the lost tooth structure. It provides the necessary bulk and shape to support a dental crown securely.

Together, these elements prepare a compromised tooth to withstand the forces of chewing, and to support restorations.

Before Post and Core Buildups


Post and core buildups are primarily used in teeth that have undergone root canal therapy. During endodontic treatment, the pulp is removed, which leaves the tooth hollow and more prone to fracture. If the remaining tooth structure is minimal, additional reinforcement is needed before crown placement.

This procedure is also used in cases where teeth have been severely damaged by decay or trauma, or where restorations have failed. The primary goal is to preserve the natural tooth, and provide a stable foundation for a crown or bridge to avoid extraction and replacement.

The Role of Posts in Reinforcement


In prosthodontics, the post serves as the structural backbone of a restored tooth. Posts are crafted from materials such as metal alloys, fiber-reinforced composites, or ceramics. The material is chosen based on the tooth's location and functional demands. Fiber-reinforced posts are chosen for their superior look and flexibility that mimics the natural look and structure of the teeth.

The post is cemented into the prepared root canal. This creates a strong foundation for the core, and distributes bite forces evenly along the root. Proper post-placement is critical to prevent stress concentration that may lead to fracture.

Building a Strong Core


The core material is layered around the post to recreate the tooth's original form. Composite resins and dental amalgam are commonly used for this purpose. In addition to providing bulk, the core must adhere to the tooth and be shaped to fit the crown accurately. The final restoration must balance strength with appearance, particularly in teeth in the front of the mouth.
